If you are user Example, and you have just checked an article for COI by user John Smith who has declared his connection at the diff with revision ID 442212827, use this:
The following Wikipedia contributor has declared a personal or professional connection to the subject of this page. Relevant policies and guidelines may include conflict of interest, autobiography, and neutral point of view. Their edits to this page were last checked for neutrality on 20 September 2020 by Example.
Use this template when dealing with a general conflict of interest, including financial conflict of interest that does not involve being paid to edit Wikipedia. It can be used whether there is an actual (confirmed) or an apparent COI, but if the COI is unconfirmed do not add it without justifiably strong suspicion.
Do not use this template solely to identify an article subject as a Wikipedian.
Parameters
|checked=
Text parameter. Most recent date that a neutral editor checked the edits of all known connected contributors for neutrality as well as the name of that editor. For example: |checked=31 December 2016 by Example
You can use templates {{today}} (substituted) and {{u}} to insert this conveniently: |checked={{subst:today}} by {{User link|{{subst:REVISIONUSER}}}}
|user=
Text parameter. User name of the COI editor. For example: |user=John Smith
Note that the user name is automatically linked complete with talk page and contributions (i.e. the {{user}} template is automatically applied for you)
|alt1=|alt2=|alt3=|alt4=
Text parameters. Alternative names for the COI editor. Useful in cases of Wikipedia:Sockpuppetry. Otherwise works like |user=.
|editedhere=
Yes/no parameter (optional). If the COI editor has edited the present article, enter "yes".
|declared=
Provide the diff number in which the COI editor has declared their connection/financial interest in Wikipedia. Can also be used as a yes/no parameter if there is no diff. Optional.
|otherlinks=
Text parameter (optional). Provide additional information about the declaration. Formerly commonly used to provide the diff link.
|banned=
Yes/no parameter (optional). If the COI editor user has been formally banned from editing the present article, enter "yes". This adds a note to that user name, informing readers that if they spot contributions of that user that weren't caught and reverted, they may do so. Please do not use this parameter if you are not sure about the COI editor's status.
|small=
Yes/no parameter (optional). If you need to make the box small and right floating, enter "yes".
Multi-listing
The above example was for listing a single COI editor.
This template is capable of listing up to 10 editors. Assign a number between 1 and 10 to each editor listed, and substitute that value for X in each of the parameters listed below. Parameters otherwise work as described for single-listing above.
UserX
the user name of the COI editor. Note there is no capacity to list alternative names (per the |alt1=...|alt4= parameters above) in a multi-listing.
UX-EH
Edited here: whether or not the COI editor has edited the present article
UX-declared
COI declared: whether the COI user has declared their connection; can take the diff if yes to create a link
UX-otherlinks
Other links: a link to the diff of the editor's COI declaration
UX-banned
Banned: whether the user has been formally banned from editing the present article
Example
{{Connected contributor| checked = 31 December 2016 by {{User link|Example}}| User1 = WikiWriter2458 | U1-EH = yes | U1-declared = 442212827 | U1-otherlinks =| U1-banned = no
| User2 = John Smith | U2-EH = no | U2-declared = no | U2-otherlinks =| U2-banned = no
| User3 = Janet Doe | U3-EH = yes | U3-declared = yes | U3-otherlinks =| U3-banned = yes
| User4 = xx | U4-EH = yes | U4-declared = no | U4-otherlinks =| U4-banned = no
}}
